چکیده

Branched-chain amino acids increase the brain perfusion of patients with hepatic encephalopathy (HE), but the amino acid and the mechanisms involved are still unknown. This study compared brain perfusion and clinical improvement during leucine or isoleucine supplementation. After randomization, 27 subjects with cirrhosis and HE received leucine or isoleucine supplements for one year. Brain single photon emission computed tomography (SPECT) and dynamic brain scintigraphy (DBS) were performed pretreatment and at 1, 8 and 12 months of supplementation. Brain perfusion was increased only in the isoleucine group at 8 months of treatment by both SPECT and DBS (p<0.001 and p = 0.05, respectively) and by SPECT at the 12 month (p < 0.05). This was associated with hepatic encephalopathy improvement at 8 and 12 months (p=0.008 and 0.004, respectively), which was not observed in the leucine group (p=0.313 and 0.055, respectively). Isoleucine supplementation achieved a better impact on brain perfusion restoration in HE. ACCEPTED MANUSCRIPT

Branched-chain amino acids in the treatment of chronic hepatic encephalopathy.

The therapeutic efficacy of orally administered branched-chain amino acids in patients with liver cirrhosis and chronic encephalopathy was examined in a double blind, randomised crossover study. Seven patients with manifest hepatic cirrhosis and encephalopathy of six months' duration or longer ingested 30 g branched-chain amino acids or placebo during two 14-day periods. Psychometric tests and ...

The effect of L-ornithine L-aspartate and branch chain amino acids on encephalopathy and nutritional status in liver cirrhosis with malnutrition.

AIM to determine the effect of L-ornithine-L-aspartate (LOLA) together with the nutritional improvement and branched chain amino acids (BCAAs) substitution, on encephalopathy in liver cirrhosis with malnutrition. METHODS liver cirrhosis patients visited Cipto mangunkusumo Hospital in June-October 2009 were evaluated by critical flicker frequency (CFF) test. Encephalopathy is defined when CFF ...

Safety, efficacy, and patient acceptability of rifaximin for hepatic encephalopathy

Hepatic encephalopathy is a complex disease entity ranging from mild cognitive dysfunction to deep coma. Traditionally, treatment has focused on a reduction of ammonia through a reduced production, absorption, or clearance. Rifaximin is a nonabsorbable antibiotic, which reduces the production of ammonia by gut bacteria and, to some extent, other toxic derivatives from the gut. Clinical trials s...

*Taste: Bitter *Properties: Readily soluble in formic acid, poorly soluble in water, and practically insoluble in ethanol. Soluble in dilute hydrochloric acid. Major effects and applications of BCAAs

In this series, we herein present our research on the physiology and usefulness of Branched-Chain Amino Acids. As often presented in the preceding issues of the journal, of the nine essential amino acids, valine, leucine and isoleucine, which have branched side chains, are called Branched-Chain Amino Acids (BCAAs). BCAAs play important roles, particularly during exercise and in the maintenance ...

Update on branched-chain amino acid supplementation in liver diseases.

PURPOSE OF REVIEW Branched-chain amino acids (BCAAs) have a peculiar role in whole-body nitrogen metabolism. BCAAs are a substrate for protein synthesis, and have been used to conserve or restore muscle mass in advanced liver disease. In addition, the competitive action of BCAAs on amino acid transport across the blood-brain barrier may improve hepatic encephalopathy. RECENT FINDINGS The effe...
